Version: 0.10.4 (using KDE 4.4.4) OS: Linux When using this pdf with forms, http://www.ppt.gc.ca/form/pdfs/pptc054.pdf (passport application from the gvmt. of Canada), after hitting <show forms> button, the form fields look fine and in place. But, once you scroll up/down using the mouse wheel or pgup/pgdn keys, all fields get misplaced all over the page. Some fields from other pages show current page also. Document becomes unreadable. Form feature becomes non-functional. Reproducible: Always Steps to Reproduce: 1. download http://www.ppt.gc.ca/form/pdfs/pptc054.pdf 2. Open with okular 3. Hit <show forms> button 4. scroll down Actual Results: form fields get misplaced all over the page. Document becomes unreadable. Form feature becomes non-functional. Expected Results: form fields should scroll together with text on the page. OS: Linux (i686) release 2.6.34.7-0.5-desktop Compiler: gcc
I get the same problem with KDE 4.5.3 on openSUSE 11.3 (x86_64). I will attach a form and a screenshot showing this.

Could the title of this bug please be changed from "From fields..." to "Form fields..."? The misspelling makes this bug harder to search for.
(In reply to comment #4) > Could the title of this bug please be changed from "From fields..." to "Form > fields..."? The misspelling makes this bug harder to search for. ha ha good catch. Bug in the bug report, ha?
Reproducible in Okular 0.13.3, KDE 4.7.3.
Fixed in 4.8.0, actually duplicate of some other bug that talks about the same situation with checkboxes or radio buttons or something. Anyway should be fixed with KDE 4.8.0, if not please reopen.
