Version:           1.13.5 (using KDE 4.4.5) 
OS:                Linux

When setting up an external editor, there should be an option to include the mail headers into the text file.  Other mail clients, e.g. mutt, do that, and there are editors that can handle headers (e.g. syntax highlighting).  Advantage: you can e.g. change the subject while composing the mail, you don't forget to whom you are writing, etc.

Thank you for your feature request. Kmail1 is currently unmaintained so we are closing all wishes. Please feel free to reopen a feature request for Kmail2 if it has not already been implemented.

In the kmail version that comes with KDE 4.9.0 (Is this kmail2?) such an option still doesn't exist.

Let me add another advantage of being able to edit headers in an external editor: You can write macros in the external editor, which do certain things based on information in the header, e.g. the receipient of the mail.
