Upon further investigation of the *rc files in .kde4/config, I discovered that all of the KDE apps had a Main Window size changed to 1 pixel higher and 1 pixel wider than my display: [MainWindow] Height 1080=1081 State=AAAA/wAAAAD9AAAAAwAAAAAAAABhAAAD/fwCAAAAAvsAAAAWAGYAbwBsAGQAZQByAHMARABvAGMAawAAAAAA/////wAAABcA////+wAAABQAcABsAGEAYwBlAHMARABvAGMAawEAAAA7AAAD/QAAAF4A////AAAAAQAAALIAAAP9/AIAAAAB+wAAABAAaQBuAGYAbwBEAG8AYwBrAQAAADsAAAP9AAABEwD///8AAAADAAAAAAAAAAD8AQAAAAH7AAAAGAB0AGUAcgBtAGkAbgBhAGwARABvAGMAawAAAAAA/////wAAAEwA////AAAGYQAAA/0AAAAEAAAABAAAAAgAAAAI/AAAAAEAAAACAAAAAgAAABYAbQBhAGkAbgBUAG8AbwBsAEIAYQByAQAAAAD/////AAAAAAAAAAAAAAAaAHMAZQBhAHIAYwBoAFQAbwBvAGwAQgBhAHIAAAAAAP////8AAAAAAAAAAA== ToolBarsMovable=Disabled Width 1920=1921 Changing these restored the title bar. I'm not sure how they got to this point however.
Does changing the size to make it match your resolution fix the issue ?
Ack Sorry, I forgot to add that, yes manually changing the size fixes it.

@graham - are you using or did you ever try or just call plasma's netbook mode? ("plasma-netbook") - using or ever tried kwin's tiling feature? (only if you're using 4.5 / as well) - did you setup (and later remove) a "blind" kwin rule to make all windows fullscreen?
@Thomas No to all three of the above. A reboot cause Dolphin to revert to the behaviour, but not the other apps. Whether this is caused by shutting down with Dolphin maximised I don't know, I shall try it later today. Is there any log files that I can access that may be useful?
- It does only hit these three applications? - Does changing the UI style / kwin decoration have any impact?
Since I fixed it I can't reproduce it with Okular. Digikam, Kaffienne, Konqueror and Dolphin however exhibit the same behaviour after reboot. Digikam and Konqueror of course can be restored using the Ctrl+shift+f keyboard shortcut. Kaffienne and Dolphin don't have that facility. Changing desktop themes makes no difference, that was my first consideration.

looks like maximized windows are affected. what's the output of "kreadconfig -file kwinrc -group Windows -key BorderlessMaximizedWindows"

Either - you're running them in fullscreen mode (either by application setting (unlikely) or WM rule), or - it explicitly sets a "no deco please" hint (just post the _MOTIF_WM_HINTS line, no human bein can read that encoding ;-) or - the WM has a bug. but in that case you're on the wrong bugtracker then ;-) -> http://wiki.compiz.org/HowtoSubmitBugReports
ok. i managed to fix it by opening the affected aplications and selecting Kwin as my window manager. the title bar appeared and i unmaximized the apps and then without closing the apps, selected compiz again as my windows manager. everything is normal now.
clear compiz bug (or some accidentally activated feature) then. @Graham, ca we assume you're using kwin? ;-)
assuming the answer is "no" @graham: if you are sure to not use compiz, but kwin, and the issue remains - please step in and revert the bug to "unconfirmed"
