official form from citizen center of Berlin, Germany

Version: unspecified (using KDE 4.4.3)
OS: Linux

I found a bug with the attached pdf file which is an official form of the german
citizen center.

When I fill up the last first line (two line edits in one line) of the last
block on the last page an leave the form mode, the content of both entries get
filled in all other lines, too. Thats not what I would expect.

Reproducible: Always

Steps to Reproduce:
open the pdf file,
go to the end of the last page,
enable form editing,
fill in from the last block the first line,
disable form editing,
see that the other lines of the last block got filled, too

Expected Results:
that all line edits are independant of each other

Unable to reproduce in Okular 0.13.3. If you can still reproduce, please provide a screenshot of the issue.

Works here too. KDE 4.4.3 is very old, please update to a newer version.
