In those cases it is necessary to rename the new image. The buttons on the rename file dialog box each have hot keys, but when a button is inactive (greyed out), the hotkeys are interpreted as regular keys. For example, if "Suggest New Name" has not been chosen, then "Rename" is still unavailable. But pressing alt-r will act like regular 'r'. Alt-r should do nothing at all until the "Rename" button becomes active.
