The viewer will display icons incorrectly for attachments with an unknown content-type:

Example:
Content-Type: application/x-outlook-tnef

Instead of the "unknown" icon, the viewer displays something that looks like the icon can't be loaded

I cannot reproduce this behaviour. When the attached file's mime type is unknown, I get a icon for binary data mime type. Maybe this should be marked as closed?

Still happens to me, see the attached mail as an example source of the bug. You can import it into KMail with File->Import Messages (choose mbox format and some target folder)

Fixed in 4.7.1 or 4.7.2 and master
