Installed from:    Ubuntu Packages

In multiple versions of KDE 4 that I tried, (from an early 4.3 pre development snapshot, to 4.4.1) And in Kubuntu and OpenSuse, the sounds under "KDE System Notifications" for questions and errors are not played. 

This is easily reproduced. 

Some instances I can think of:
with the question sound set to play a sound in the KCM: 
Open Kate, type a few characters, and close out the window. The do you want to save dialog plays no sound

Open systemsettings, and make a change in a KCM without hitting apply. The dialog prompting to save the settings plays no sound

Open konsole, or konqueror, and open two tabs, and close the windows. Both dialogs play no sound.
