Steps to reproduce:

-Have your desktop unlocked
-Add an icon to launch a program, e.g. kile, to the panel
-Now update the corresponding rpm in my case, while running Plasma-Desktop
-Notice that during the update the icon is removed from the panel.

I checked that the corresponding entry for the icon is really removed from plasma-desktop-appletsrc.

I first recognized this after the update to KDE SC 4.4.1, if I remember correctly.
It does not happen if your desktop is locked.

This should be fixed in KDE SC 4.4.2 (as the file deletion notification waits 5 seconds to confirm it was properly deleted.)
When a package is updated, the package files are removed and created in quick sucession.. and that was causing KDirWatch to recognize only the file deletion (and therefore the Icon in the panel was deleted)
Fixed by David Faure IIRC
