OS:                Linux (i686) release 2.6.32.3

That's really funny that all PDFs from http://www.cacr.math.uwaterloo.ca/hac/ are printed (duplex, long side) except http://www.cacr.math.uwaterloo.ca/hac/about/chap10.pdf .

Does print preview show you something?
Which poppler version are you using?

A preview shows 41 empty pages.
Here are the installed packages at my stable Gentoo :

As preview works for me and i'm using a newer poppler i'll assume this is your problem. If you update to poppler >= 0.12.3 and still do not see anything in the print preview dialog please reopen this bug.

yep, w/ poppler-0.12.1 okular works fine.
