What I was doing when the application crashed:
After a crapy update I wasn't able to access Phonon menu in System Settings and I tried to kill System Settings manually from System Monitor!

This could be related to bug 214836 or bug 196350.
It seems you killed the Kwin helper dialog that warns about freezed/hanged applications. - Can you confirm that ?
