Currently Kmail handles those two actions as one combo with very confusing name. Please provide the same action as separate entities, so user could add to toolbar (just an example)
mark as ham
separator
mark as spam

Please look carefully. KMail has both an action with a drop down menu for the filters, as well as separate actions for "Apply filter 'Mark as Spam'" and "Apply filter 'Mark as Ham'".

My KMail toolbar always had both actions separately after setting up the SpamAssassin tool.

There is no such entry as "apply filter "mark as spam"" (or ham). 

There are: apply filter, apply all filters, ActionList: toolbar_filter_action.

I don't use SA but bogofilter (another story if this is really dependant of used filtering mechanism it should not be).

There are seperate "mark as ham" and "mark as spam" buttons installed by default after having configured a spamfilter.

I am not talking about what is (or not) installed by default, but how it is available. Check your configuration -- probably you see a button "classify as spam" in the toolbar, but in toolbar configuration you won't see such entry (at least I don't see it -- KDE 4.4.0).

Fixed in kmail2, two icons are created after running the spam wizard and are available in the toolbar settings.
