Version: (using KDE 4.3.0) OS: Linux Installed from: Ubuntu Packages Kate still suffers from Bug 168480. When the icon size is set to default (16px in my case) the icons are reset to 22 after restarting Kate.
This bug is still present in 4.3.1.
And still present in 4.3.2 as well.
Still present in 4.3.5.

Not present here. I set the Icon Size to 48 to Main Toolbar from System Settings, after applying the icons themselves in System Settings get the new size and the one in Kate as well. My kdeglobals file has a line Size=48 in [MainToolbarIcons] category. Screenshot will be attached for Kate

And if I quit Kate and restart it, the icon size stays as specified in System Settings.
So, could you try 16x16 for Maintoolbar? That's what I have in kdeglobals under MainToolbarIcons.
Ok, problem solved. There was a stale default.katesession in my .kde/share/apps/kate dir, that I didn't know existed. I configure kate to start a new session every time I start it, and somehow this defaultsession got saved one time and was never removed. My problem has been fixed and I'll (shamefully) close this bug.
Yes it works the same with 16. Another thing I can do is leave global settings to 22 (default) from System Settings (Application Appearance -> Icons -> Advanced tab) and set only Kate toolbar icons size to 16 (right click on Kate toolbar and choose small 16. If I quit and restart Kate the toolbar icons size is still 16. The katerc file has the following lines: [MainWindow][Toolbar mainToolBar] IconSize=16 Check your katerc file please.
OK! :-)
One last remark and I'll repent in my little corner for the rest of the year: It's a bit confusing to say the least, that (Kate?) sessions restore iconsettings in my very humble opinion. So I'd be inclined to call this a 'bug' and not a 'feature'.
