Version:           1.11.4 (using KDE 4.2.4)
OS:                Linux
Installed from:    Slackware Packages

When opening (clicking) an email in the message list which contains a few (eg. 5) image attachments, KMail hangs, and the cpu load goes to maximum. The gui becomes responsive again after ~1min when KMail opened the email, and then I can see the images also.

Attachments displaying is "Smart".

I guess this is a duplicate of bug 191666.

yes, probably. Please retry with KDE 4.3 RC3 or higher.
