I can confirm this. Currently using KDE 4.2.90 (Beta3) Kubuntus PPAs. The funny part about this is, when i print to PDF through cups (cups pdf filter), the layout seems to be okay. But printing to a "real" printer has the wrong orientation, its always landscape. Cups is at version 1.3.9-17ubuntu3.1.
I can confirm that orientation works well when using CUPS
Okular prints a portrait PDF A4 document as landscape. But when it shall print portrait, it prints landscape. The problem seems to be a confused checkbox landscape/portrait. The applied printer is HP 1018.

I'm seeing this bug in kde 4.3.1 on debian squeeze. Okular and xpdf print incorrectly (printing in landscape mode always) via a cups printer (the printer is a Samsung ML2851ND network printer). Prints fine from kpdf, and other applications print fine too (konqueror prints web pages e.t.c)
Workaround has been found. If the Job Option "Scale to fit" in system-config-printer-kde is deselected the problem goes away!

i think it works in 4.9
Debian Squeeze 2.6.32-5-amd64, KDE 4.4.5, Okular 0.10.5, system-config-printer 1.2.3, HP LaserJet printers (HPLJ-6L, HPLJ-cp2025cdn). Workaround under printer Properties/Job Options unchecking 'Scale To Fit' has no effect nor does changing Orientation to/from 'Automatic' or 'Portrait (no rotation)'. If PDF is in Landscape orientation, printing from Okular, and selecting the printer Properties shows 'Landscape' correctly, but prints in Portrait. Workaround for me is to select 'Portrait' orientation under printer Properties. This has to be done with each print job (PITA).

Have another system: Squeeze 3.2.0-4-amd64, KDE 4.8.4, Okular 0.14.3, that works fine. Just wanted to add to the body of knowledge on what systems it does NOT work on. now readers will also know it works (at least for me) on a KDE earlier than 4.9, although I don't know if it is an Okular issue or KDE.
I have three boxes with opensuse 12.3, two of them have Okular 0.19.3 with KDE 4.13.3 installed. Only no. 3 has Okular 0.20.2, KDE 4.14.2. I also have two printers: A Toshiba e-Studio 151 and a Brother HL-5250DN. Only no. 3 suffers from the always-landscape problem and only when I print PDFs on the Toshiba. I have triple-checked all settings regarding orientation. I could not find the job option "Scale to fit" in system-config-printer-kde (as of comment #6). Maybe that got removed?
