Version:           0.8.4 (using 4.2.4 (KDE 4.2.4), Debian packages)
Compiler:          cc
OS:                Linux (i686) release 2.6.30-rc8-hope

If i open a file (using the normal open dialog), it will not appear in recent files list.

And in your case? Did it work in previous versions? Any other detail...?

It wasn't working also in the previous version in debian (should be kde 4.2.2). Well not working 'always', but time to time it does. I couldn't find a pattern.

Can't find a pattern, but still doesn't work 100% of the times.

Maybe the problem arises whenever many instances of okular are open at the same time?  Then the last instance of okular saves the list of "recent files".  All files opened by other instanced are overwritten.

Is this bug still reproducible in the latest version of Okular? If so, could you confirm whether the suggestion in #6 is accurate or not?

At least the issue I mentioned is still there. To reproduce:
Open two different files (say, A.pdf and B.pdf) in two instances of okular.  Close first the okular window with A.pdf, and then close the other okular window.  Then, when starting okular again, the list of recent files contains B.pdf, but not A.pdf.

I just went over the steps to reproduce this bug by using multiple instances of Okular and it's still happening under Okular Version 0.17.1, KDE 4.11.1. +1 to comment #6.
If you only open one document at the time, open recent list is displayed just fine, no bug.
Also, if I open a pdf file by using the File->Open option, the doc is shown in the open recent list too.
The question is: Is this bug for the original statement "If i open a file (using the normal open dialog), it will not appear in recent files list." or for the problem with the open recent list while using multiple instances of Okular?

By the way, I believe this bug report should be closed and pay attention to Bug 265218 - The "recent documents" file menu should be consistent across instances.
