Especially for new users switching to KMail, it is a little off-putting not to have a reply-all button on the toolbar, especially since this is perhaps the most commonly used function of an email client. I know that it's only two clicks to add it to the toolbar, but it should really be there by default.

This is not necessary. The reply button already offers this choice when you don't release the mouse button immediately.
