Version:           1.11.1 (using KDE 4.2.1)
OS:                Linux
Installed from:    Ubuntu Packages

You can drag'n'drop columns in KMail to place it in desired order - for example, i place them in following order: Attachments/Sender/Subject/Date.

After restarting KMail all columns returned to thier default order, as in context menu Show Columns.

I think this is very huge bug (not feature request!) related to usability and should be fixed ASAP.
