I have been using nvidia-settings to setup my xorg.conf so I would, under kde 3 and gnome, have 2 screens with 2 different X running. But with kde 4 I have my primary screen working as expected and the 2. screen is gray. On the 2. screen I can only move the mouse curse around but nothing happens if I press left or right on the mouse.

Workaround: 
use one X to both screens but this is to preferable since some programs (at least glob2 and WoW using wine or crossover) thing that the the x is the whole desktop and therefor, running them full screen, makes them cover both screens
