 Using this button only makes sense when the <action> button (see above) is an 'ok' button.

And <action> button:
Mutually exclusive with Apply

I have no idea what you are talking about.  Can you provide a better description and an example screenshot?

It is description from HIG, previous version, about dialog buttons naming convention. It is hard to say it is fixed now, because in new HIG there is no word about buttons names.
