Here using: Qt: 4.5.0 + qt-copy-patches-936035 KDE: 4.2.68 (KDE 4.2.68 (KDE 4.3 >= 20090327)) kdelibs svn rev. 949044 kdepimlibs svn rev.949046 kmail svn rev. 949046 on ArchLinux i686 - Kernel 2.6.28.8 I can't reproduce this. With "Load attachments on Demand" disabled, the icons of the attachments are shown properly for new and old mails.
i think even in 4.2.2 it works now with "load attachment on demand" checked.

Version 1.11.2 Using KDE 4.2.2 (KDE 4.2.2) "release 111" today again I found a mail from yesterday - which I received first in my office - has no icons on my home office computer
Thank you for taking the time to file a bug report. KMail2 was released in 2011, and the entire code base went through significant changes. We are currently in the process of porting to Qt5 and KF5. It is unlikely that these bugs are still valid in KMail2. We welcome you to try out KMail 2 with the KDE 4.14 release and give your feedback.
KMail2 had similar bug #286692, it's already fixed.
