Kmail filter.
The header was
To: "'xxx x xxxx'" <xxxxx@xxxx.com>, texhax@tug.org
The filter was
To  contains  texhax
The match failed: the logfile read
[15:48:10] 0 = "To" <contains> "texhax" ("xxx x xxxx" <xxxxx@xxxx.com>)
i.e. it seems the second address on the header line was ignored.   procmail would have matched it, IIRC.

list texhax@tug.org.]

I can't reproduce on 1.11.2. Works fine with 2nd to address.
