The xface could be used as a main personalized emoticon to show the current emotion towards the topic of the email. So the composer would optionally allow to pick a certain xface for the composed email. No idea whether to do this per identity or global.

Thank you for your feature request. Kmail1 is currently unmaintained so we are closing all wishes. Please feel free to reopen a feature request for Kmail2 if it has not already been implemented.

xface and the replacement face header are unfortunately a bit outdated for today needs (48x48 pixel image is not a lot) and I'm not even sure a lot of clients outside of KMail support it. The current way to set user avatars is via libreavatar. I don't think we will invest more time in x-face and no one did implement this in 18 years, so closing this feature request.
