IMHO for saving such a file, the original name should be restored and the default location should be proposed. it does not make sense to store the document in the temp area. if the problem could be solved in the kfile module it could be also used by OO and others. all file names seem to start with: /tmp/kde-<LOGINNAME> and end with: _[<random extension>].<doctype> so converting to a meeningfull name should be possible.

I don't understand. We have to append a random extension to the file name to prevent problems with equally-named files. Imagine you want to open two different files which for whatever reason are both named brief.doc. What do you mean by "saving such a file"? Saving from where? Saving from within KMail is working as you say for me. Or do you mean saving from within, let's say, OpenOffice.org? How are we supposed to change the location and/or filename OpenOffice.org proposed if you save the document from within OpenOffice.org?
open an attachment, example pdf or doc file. it gets the necessary extension as you explained. you want to save it out of this application (to make it editable - kword, OO). (instead of cloosing it, saving the attachment out from kmail and open it from the new location) in this case the pre and postpended parts of the file name could be removed again to recover the original file name. the default (or last) location for saving files should be used, but never /tmp/kde-<username>
In Bug #107958 I suggested opening its own (randomly named) folder for each mail in /tmp. So you won't have the problem with doubled names.
