Version:           1.9 (using KDE KDE 3.5.1)
Installed from:    Compiled From Sources
Compiler:          gcc version 3.4.3
OS:                Linux (i686) release 2.6.15.4-0

There are two parts to the same bug. To reproduce:

Start KMail.
Go to Settings->Configure KMail...
On the Identities pane, click Modify...
Make no changes. Click 'OK'.
Back on the parent configuration dialogue, the 'Apply' button has been enabled, even though there should be no changes to apply (this behaviour is contrary to the universal convention that an 'apply' button is only enabled when clicking on it will have an effect).

The second part:

Now click 'Cancel' to close the settings dialogue.
Go back to Settings->Configure KMail...
'Apply' is still enabled. It should be disabled.

Thanks for reporting Hugh. I guess the first part is sort of OK, because clicking OK is in a way "modifying". That's what implicitly happens with "Configure KMail..." too, relevant parts are reloaded when you click OK for the main configuration dialog, even if you don't make any changes.

I agree that the second part is a bug.

second point works fine with KDE 4.3
