When using spell check in kmail, there are multiple ways of selecting the spell-check language. Two notable ones are:

1) as part of the "identity" profile
2) using the settings->spellcheck menu item while typing a message.

The second option is particularly useful if it happens to you to write an email in a language that is not your own (default) language (eg. you are Italian, but you want to write a msg in English).

Unfortunately, the "as you type" spell checker seems to ignore option 2.

Confirmed. The 3rd way (View, Dictionary) works fine though.
