I am trying to access pages using https:// protocol from behind proxy. Windows server which is proxy is SBS (Small Business Server).
Konqueror does not ask for authentication and the following page appears:
The page cannot be displayed
  There is a problem with the page you are trying to reach and it cannot be displayed.
   
Please try the following:
Click the Refresh button, or try again later.
 
Open the Web site home page, and then look for links to the information you want. 
If you typed the page address in the Address bar, make sure that it is spelled correctly.
 
Verify that the Internet access policy on your network allows you to view this this page.
If you believe you should be able to view this directory or page, please contact the Web site administrator by using the e-mail address or phone number listed on the Web site home page.

When I open the same page using Firefox, authentication dialog appears and after providing username and password I can view the page.
It is also not the problem of setting proxy in Konqueror preferences, as this is already set. I can browse normal pages (using HTTP) in Konqueror using this proxy.

Please, could you re-test against the newer Konqueror version like 3.5.9 and 4?

Although this problem have been reported for KDE 3.4 , I now get the same problem with KDE 4.2 with Mandriva Cooker. Until KDE 4.1, I did not have this kind of problem : I have always been asked the password for my proxy server, but now I'm not. In addition, the identification box in konqueror is greyed out, see the attached image.

Additional information : it seems libkntlm doesn't work properly. Here's what I get in my ~/.xsession-errors :

I can confirm this in Konqueror 4.5.1. Have to use Firefox meanwhile :(

I'm not having this problem anymore with KDE 4.6 beta. Can anyone else confirm it?

This should work fine now. I spent a good couple of fixing NTLM related bugs with a generous and patient user who provided access to his servers for me to test against. Anyhow, if you still have such problems with KDE 4.6 and up, feel free to reopen this ticket.
