I can reproduce this reliably in konqueror and PSI (yeh i know PSI ain't KDE).
1) enable "Focus Follows Mouse": Control Center > Desktop > Window Behavior > Focus > Policy
2) maximize anything (so that there is a window to lose focus too)
3) Open konqueror
4) place the konqueror window so that clicking on a file at the bottom of it will result in the rename option from the menu not overlapping the koqueror window
5) click rename
6) notice the fact that typing will not edit the file
