Version:            (using KDE KDE 3.4.0)
Installed from:    Compiled From Sources
Compiler:          gcc version 3.3.5-20050130 (Gentoo 3.3.5.20050130-r1, ssp-3.3.5.20050130-1, pie-8.7.7.1) 
OS:                Linux

When I go to the Help->Report Bug link in any KDE application, I am directed to ...

/var/tmp/kdecache-lee/krun/28304.0.wizard.cgi

The form that comes up looks sort of like the bug report website, but it is not really, and an error page comes up when the form is submitted.

"An error occurred while loading file:///var/tmp/kdecache-lee/krun/wizard.cgi:
The file or folder /var/tmp/kdecache-lee/krun/wizard.cgi does not exist."

Looks like your Konqueror configuration or text/html type association is broken. Make sure it reads "konqueror %U" or "kfmclient openURL %u"

text/html has "kfmclient openURL %u text/html" as the first listing, which is the same as another, working system that I use.

I tried changing it to just "kfmclient openURL %u" but have the same result as previously.

When I got your message, I looked at the association for text/html and it was as you said, but it was not working.

Just now, I looked again, and the Konqueror entry was gone.

Adding in a new Konqueror entry with application "kfmclient openURL %u" fixed the problem.

I am not sure what was causing it, but if someone has this problem in the future they might try removing and re-adding the Konqueror entry for text/html.

I started getting this error again. I can fix the problem, but something is changing this file association without my input.

Right, but we don't have code to change the association without your input.

Either it's a program you installed, or maybe you associated it unknowingly when an Open With window opened, and you left "Remember" checked.

I'm sorry, but the original report is no bug. If you have a problem about associations changing, and you think it is a bug, file a new report.
