Version:           3.4.0 (using KDE 3.4.0, Gentoo)
Compiler:          gcc version 3.3.5 (Gentoo Linux 3.3.5-r1, ssp-3.3.2-3, pie-8.7.7.1)
OS:                Linux (i686) release 2.6.10

Upgrading from 3.3 -> 3.4, my konqueror toolbar customizations were lost. Now I am trying to put things back to right.

I open the Settings->Configure Toolbars... window.

When I click on an "Available Action" on the left and click the right arrow to move it to "Current Actions" it disappears from the left, but does not appear on the right. Poof!

Just out of curiosity ... moving actions from current to available also causes the action to disappear completely.
