SUMMARY
Filling the pdf that you can get from the Polish government:
https://bip.malopolska.pl/e,pobierz,get.html?id=3306136&file=undefined
you can see data when you fill it, but when you disable 'show form' the form is blank

STEPS TO REPRODUCE
1. Download  pdf https://bip.malopolska.pl/e,pobierz,get.html?id=3306136&file=undefined
2. open with okular 
3. click on `show form`
3. Fill surname 
4. click again on `show form`
OBSERVED RESULT

The text of the form will be invisible

EXPECTED RESULT

The text should be rendered 

ADDITIONAL INFORMATION

I tried before with okular 22.12.3 from debian stable, and after I install also okular (24.02.0) from flatpack and get the same result
