Are you using the "remember settings per display arrangement" setting on the Display Configuration page in System Settings? Can you paste the output of `kscreen-doctor -o` twice: once after you set the correct resolutions, and again after you reboot and they've gotten scrambled up? > Are you using the "remember settings per display arrangement" setting on the Display Configuration page in System Settings?
I'm using "For any display arrangement" in the settings. But it doesn't sound like that should make all HDMI display settings the same. Is this intended behavior?

So I'll close this issue. If I see it happening again, I'll report back I happened again. I didn't change any configuration, but now my 1440p monitor is outputting at 1080. It seems that it happens when I connect to a 1080p display that my laptop has not seen before. But that's just a guess. Thanks for the info. Is it happening on Wayland too? Or only on X11?
