Works for me. Can you reproduce the issue by restarting plasmashell with `plasmashell --replace` in a terminal window or KRunner? If so, can you attach a screen recording that shows the issue? Created attachment 140531 [details]

This is right after a reboot.
Right click on widget > Refresh view - will not change the icons.
Right click on icon > Properties > OK - used to refresh it, now I need to do what is in this video. After every reboot it will revert back to the regular icon at the start of the video.
The "notes" icon is a symlink to a directory in the home folder so it is definitely mounted. There is a chance the "anet_a8" link was not if that helps.
Ah, I can reproduce the issue now. Indeed the icon is being displayed incorrectly after plasmashell is restarted. However the custom icon you chose is still set; something is simply not reading it right on startup.
