Is there anything to suggest the issue exists at a plasma level rather than in pulseaudio? I don't really know. Is there a way for me to find out? I encountered something similar once. It was due to pulseaudio's switch-on-port-available module trying to mute the volumes. If this is the same thing, can you say if commenting out the "load-module module-switch-on-port-available" line in /etc/pulse/default.pa fixes this for you? I commented out the line you mention and reboot but it didn't fix it.
