STEPS TO REPRODUCE
1. open dolphin
2. open new instances by pressing ctrl+n several times in sequence
3. 

OBSERVED RESULT
As we can see in my screenshot, some windows are grouped, others are not

EXPECTED RESULT
all windows of a same app are always grouped

Additional info: the entry of a non-grouped window moves itself to its group when clicked.

Apparently it's fixed. I'm unable to reproduce with Dolphin and Konsole on Plasma 5.23 beta.
