For files without any association (unknown extension and unknown mime type) there is no "Add file association" or "remember association" setting in the "Open" dialog. However, there is a button in the right-click "Properties" dialog. Also visible in the third screenshot. a@kkrz.tk, have you tried this? And the dialogs are part of KIO frameworks, not Krusader. Problem exists only in Krusader. Please try Dolphin, not Nautilus. Dolphin uses the same KDE framework under the hood. @Jake, could you please try in Dolphin as Toni and Alex suggested and report back?
