For browsing long web pages I find that the IE behaviour where you can click the mousewheel and automatically scroll the page by moving the mouse until you click again on the mousewheel is very easy on the wrist.

If this could be done without breaking the "open in new window" behaviour for non-mousewheel middle buttons that would be cool.
