The Recently Used list in the application launcher shows a list of files you have recently opened.  However, this only seems to include files accessed through the file manager (or file picker dialog).  It does not include applications opened through an application's own "Recently Used" list.  

Reproducible: Always

Steps to Reproduce:
Open okular.  Select the File Menu -> Recently Used and then open a file that does not appear in your "Recently Used" list (if necessary open a bunch of other files to push it off the list).  Then close okular.

Actual Results:  
The file you opened does not appear in the Recently Used list.

Expected Results:  
The file does appear in the Recently Used list.

It isn't just okular, this happens with Kate as well.

Hello!

This bug report was filed for KDE Plasma 4, which reached end-of-support status in August 2015. KDE Plasma 5's desktop shell has been almost completely rewritten for better performance and usability, so it is likely that this bug has already been resolved in Plasma 5.
