okular displays a pdf correctly, but in print preview, it displays the page shrunk down to new margins. It prints the document just as shown in the print preview which is incorrect. kpdf displays/previews/prints correctly. Acroread also displays/previews/prints correctly. So I think this is something okular is doing (and not ghostscript).

This tarball contains a latex source files "bug.tex", "bug.data", "BHO.tex", as well as the compiled pdf. The pdf can be compiled by running "pdflatex --shell-escape bug.tex"

Just tried with KDE 4.6.5 and it seems to work for me, could you give it a try again?
