PDF showing the problem described.
To confirm bug, open pdf in Okular. The text in the form field will be rendered correctly. Then hit "Show Forms" and put the cursor in the form field You don't need to actually make any changes. Then hit "Hide Forms." The text now appears oriented at the right side of the form field, rotated 90 degrees to the right.
I can reproduce the same problem with Evince, a document viewer which uses the same library Okular uses for PDF documents, called Poppler. Please report the problem (so it is tracked properly) to the bug tracking system of Poppler, at https://bugs.freedesktop.org, "poppler" product. Don't forget to mention with Poppler version you are using, and the testcase. Thanks! This poppler bug is very annoying and still exists. For me it happens also with the first line I think (looks like single-line fields, but maybe a misuse).
