KMail has suddenly started sending mail beginning directly with the DATA command 
skipping completely the RCPT TO: part so the server logically returns an error

Where do you know that from?
Did that happen only once or does it happen always? If it happened only once 
maybe a network package get lost somewhere in the internet.

If there is a mail in the outbox that can not be sent KMail tries to send it 
the next time again. This problem is mentions in the documentation.
