When opening a file in Konqueror, you have a choice: open with default application, open with a list of compatible applications, or open with whatever command-line you provide.

In kmail, only the 1st and 3rd of these options exist. If you want to open in any of the applications known by KDE to be able to handle the filetype except the default one, you have to choose it manually from a chooser.

I suggest that when you right-click an attachment, kmail should have a full "Open With" submenu that is populated with the same applications as in Konqueror.
