Forms failed to render while enclosed in "pre" tag

Unfortunately, the bug only happened while using a sensitive page in our
intranet. by removing the <pre> trags from the page, it worked properly.
I have not experienced this problem since, but neither have I found a
page using pre tags to test it on since.

Testcase template

I attached simple testcase where form is inside <pre>-tag and it works fine.
Could you modify this testcase so that this bug can be reproduced.

Can't reproduce any many months since reporter has responded.
Reporter can re-open this bug if it's still a problem.
