Version:           3.0.0a5 (using KDE 3.1.2)
Compiler:          gcc version 3.3 (Mandrake Linux 9.2 3.3-2mdk)
OS:          Linux (i686) release 2.4.21-0.13mdk

It seems after some trial and error that the C++ block comments are triggered after selecting several lines, and not having the cursor on the first position... (I'm still not sure...)
And C comments are triggered in all other cases.

Would it not be better to have two menu alternatives, one for C-style and one for C++-style. Alternatively there could be an option in the configuration which would select one or the other type.

kate now uses C++ comments in all possible cases in favour of the c comments ins c++ 
mode
