A nice interface to customise aspects of code generators such as the header file or replaceing 
  include FILENAME;
with 
  require_once FILENAME;
for example

I believe that this wishlist item has been met by the new code generation system. You can now, for example, use the code viewer to customize various bits of generated code, and, furthermore, there are many more choices in the settings tab/page to allow the user to better control code generation. I suggest that more specific requirements/wishes be opened as specific items.
