This is an old bug that is becoming more important as mouse pointers grow in response to high resolution screens (and failing eyesight).  

Tooltip placement does not respect the size of the mouse pointer (and possibly the size of the desktop font).  When a large pointer is configured or worse a huge pointer like the very cute "tuxcursor"

then the pointer obscures the first word of the tooltip, making it less of a tip and more of a cryptic puzzle.  

Different parts of the desktop are affected differently.  

A good example of bad placement is with pointer over the top left corner of a Konsole window, almost the entire tooltip is hidden.  Most if not all applications mis-behave.  

It also affects Gnome.

So it might be an X problem rather than a KDE problem.
