Not all menus are affected but some that are include check spelling and 'move to' menu. To check this, open a blank email, enter some text with one or more words misspelled, then right click on the misspelled word.
I don't know what you mean. Can you provide a screenshot? I haven't seen anything strange in the spellcheck context menu. Did you try with another style than Oxygen?
After further investigation I have found this is not restricted to KMail. It also affects other KDE applications (such as Konqueror), it is just it is more apparent in KMail.
Please could you attach a screenshot of this bug so that we can attempt to reproduce it.

David, what widget style are you using?
I was using LaOra-Qt, if I change to another style the problem goes!
I *think* that this is a Mandriva only issue then, and should be reported to them. I'm just waiting for a Mandriva developer to confirm that is correct before I close this report.
