Version:           1.5.8 (using KDE 3.5.9)
Installed from:    Mandriva RPMs
OS:                Linux

1) Ctrl+Z is pressed after creating a new Class or Association (between classes) is created in a Class Diagram.
2) Close/Open the XMI File you are working with.
3) There are duplicated Folder/Classes in the Logical View.
