Version:           1.9.6 enterprise (using KDE 3.5.9)

The [General] section of kmailrc contains this line:
folders=$HOME/.kde/share/apps/kmail/mail

For some reason it didn't change when I switched to kde4 (even though I started with clean config and imported all the mail), and then... I deleted ~/.kde, assuming everything I needed was in ~/.kde4. So I lost all my mail.
Could kmail be changed to always use $KDEHOME/share/apps/kmail/mail, rather than rely on an (unreliable) config setting?

I've just tested this with a clean config, and for me, the entry was folders=$HOME/.kde4/share/apps/kmail/mail, so that works.

And for old configs, that is the expected behavior, since your old config file is in .kde, together with your old mail.
