I have a 4 year old toshiba laptop (Satellite 1900-102) with a video card ATI Radeon Mobility M6. On uindous I can have dual-head (screen divided in the 2 monitors) mode with no problems. 

I have ubuntu 7.04 beta installed. I've intalled kubuntu-desktop to have all the kde also on it. I restarted, and after login in kde (3.5.6), I went to "System settings > Monitor and Display". 

I Connected the external monitor to the laptop, and thus I saw the same screen on both monitors. 

Then in "Monitor & Display" I selected dual head mode (going into administrative mode),, played a bit with the settings to match resolution 1074+768 on both screens. And applied changes. 
It told me something about logging out the session in order to restart the X server, and so I did. But no way. Screen is cloned on both monitors. I restarted,. and same result.

I'd love to have dual head mode (as I'm used to work with under uindous on the same computer). This would be one good reason to migrate from gnome to kde for me...

Mine problem is similar too, I have a nvidia card in my laptop, with two "separate screens", not one extension of the other.
I restarted X, and the other screen is even not detected in system settings -> Display despite it is recognized well in nvidia-settings.
I want to say that this is a regression bug because it worked in kde 3.5.x series.

Hello! Sorry to be the bearer of bad news, but this project has been unmaintained for many years so I am closing this bug. Kcontrol has been replaced by System Settings in Plasma. Please give the latest version of that a try, and open a new bug in "systemsettings" if you continue to have an issue. Thank you!
