Using xvncviewer, there is the possibility to force a screen refresh by pushing F8 and "Screen refresh".

This is quite usefull in slow lines when auto refreshing isn't enought which makes krdc almost unusable in this cases even if lower quality is selected :-(

Standard disclaimer: KRDC has been unmaintained for a very long time now and because of this the future of the program is unclear.

Do you see this as something that should be a "refresh" button, or some keystroke(s), or some other method?

Just add an action for this so users could simply assign a hotkey they like. F5 sounds like a suitable default for me.
